For current patients

What is changing at Mountain Peaks, and what it means for you.

If you are already a Mountain Peaks patient, this page is written for you. It answers the practical questions first and explains the reasoning second.

The short version

  • Mountain Peaks is moving to a Direct Primary Care membership model.
  • The same physician and team continue to care for you in the same office.
  • Everyday primary care will be paid for by a monthly membership instead of billed to insurance.
  • The change takes effect December 1, 2026. Current patients have been notified directly with enrollment details. If you did not receive a notice, please contact the office.
  • You should keep insurance coverage for hospital care, emergencies and specialists.

Every question, answered

What is changing?

Mountain Peaks is becoming a Direct Primary Care practice. Everyday primary care will be covered by a monthly membership paid to the practice rather than billed to your insurance company for each visit. Mountain Peaks will not bill insurance for membership services; providers we refer you to may bill your insurance as usual.

Why is Mountain Peaks changing?

Because the way primary care is paid for had begun to limit the medicine we want to practice: shorter visits, more paperwork and less continuity. Membership funds longer appointments, easier access and time to look at the whole picture. This is family medicine made personal again, not healthcare made exclusive.

When does the change take effect?

The membership model begins December 1, 2026. Current patients have been notified directly with enrollment details. If you did not receive a notice, please contact the office.

What do I need to do?

If you want to stay, enroll through the secure enrollment system using the instructions in your notice. If you did not receive a notice, contact the office. If you would rather not join, no action is required, but please read the section below on what happens next and on transferring your records.

What does membership cost?

Adults up to age 64 pay $99 per month, adults 65 and older $110, and children $25 under a parent's membership; a household membership is $299 per month. There is no enrollment fee. See the Pricing page for details.

What is included?

Routine visits, preventive and wellness care, acute and chronic care, direct communication, and same- or next-day availability when clinically appropriate, with no copays for included services. See What's Included.

What happens if I do not join?

If you choose not to enroll, your last date of care at Mountain Peaks is December 1, 2026. Written notice of this date has been sent to all current patients. Until then we remain your physician's office: visits, refills and urgent needs are handled as they are today. After December 1 the practice will no longer be contracted with insurance companies, so care cannot be extended past that date for patients who have not enrolled, including care that is still in progress. We encourage you to establish care with a new primary care physician before December 1, and we will transfer a copy of your records to that practice at your request so your care continues without interruption. If you need help finding a new physician, please contact the office.

How are my medical records transferred?

Your medical records belong to you, and a complete copy stays securely on file with the practice whether or not you enroll. The simplest route is to have your new practice request them: most practices send us their own records request on your behalf, and once we have it your records are sent securely to the receiving practice, usually within 10 business days. There is no charge for sending records directly to another healthcare provider. If you would like a personal copy for yourself, come in and sign a release of information form at the front desk; a $15 fee applies to personal copies. Call 801.724.9840 with questions about a records request.

How do I reach Dr. Durrans after hours?

Call the office line at 801.724.9840; outside office hours it forwards to the on-call phone. Members can also message through the member app, and Dr. Durrans will respond as soon as is reasonable. For anything life-threatening, go to the emergency room or call 911.

How should I handle my insurance?

Keep appropriate coverage. Membership pays for primary care here; it is not insurance and does not cover hospitalization, emergency care, specialists or services outside the practice. Providers we refer you to, including laboratories, imaging centers and specialists, may bill your insurance as usual. Mountain Peaks cannot advise you on which plan to buy.

I have Medicare. What applies to me?

Membership works the same way for every member, whatever your coverage, including Medicare and Medicaid. Mountain Peaks is a membership practice and does not bill insurance, Medicare or Medicaid for the primary care included in membership. Providers we refer you to, such as laboratories, imaging centers and specialists, may bill your insurance, Medicare or Medicaid as usual.

I have Medicaid. What applies to me?

The same applies to Medicaid and CHIP. Membership pays for primary care here; providers we refer you to may bill Medicaid or CHIP as usual.

Why we're changing

“I became a family physician to know my patients over time, not simply treat one concern and move to the next. Direct Primary Care gives us more time to listen, respond sooner and care for your health in the context of your life.”

Owner and Lead Physician

If you have been coming here for years, thank you. The intention behind this change is to give you more of what brought you here in the first place: time, attention and a physician who knows your history.
