Pricing

One monthly price. Written down, in plain numbers.

Membership is billed monthly.* Example: two parents and two children pay $248 per month in total; a household membership covers up to two parents and four dependent children for $299. Outside a household membership, dependents are covered through age 26: children through age 17 pay the $25 rate, and dependents 18 and over pay the $99 adult rate. Larger households should contact the office for information. Membership payments qualify as an HSA expense, and FSA funds may qualify as well; check with your plan administrator.

Prepay bonus

Prepay 12 months and receive a 1-month discount: pay for 11 months and get the full year of membership.

Mountain Peaks does not use silver, gold or platinum packages. There is one membership, priced by age, and children join under a parent's membership.

What the price buys

More than a copay ever did.

With insurance, a copay bought you a slot. Membership buys the primary care itself: the visits, the follow-up and the conversation in between, with nothing due at the desk for included services.

Bills after a routine visit

Routine primary-care visits

As many as your care requires, with no per-visit charge.

Preventive and wellness care

Annual physicals, well-child checks and the check-ins that catch things early.

Acute and chronic care

The sick visit this week and the ongoing condition followed over years.

Direct communication options

Reach the practice between visits. A question does not have to become an appointment.

Same- or next-day availability

When clinically appropriate. When you are sick, waiting a week is not care.

No copays for included Mountain Peaks services

Nothing collected at the desk and no bill after a routine visit.

The member app

Schedule, message your care provider, and view results and prescriptions from your phone.

Terms, in plain language

Cancellation and re-enrollment

Membership is month to month with no long-term contract. You may cancel with 30 days' notice, or before your next billing date. Re-enrollment is always welcome; a $100 re-enrollment fee applies.

HSA eligible

Membership payments qualify as an HSA expense: you can pay your Mountain Peaks membership from a health savings account. FSA funds may qualify as well; check with your plan administrator. Members should consult their own tax advisor about their individual circumstances.

Medicare and Medicaid

Membership works the same way for every member, whatever your coverage, including Medicare and Medicaid. Mountain Peaks is a membership practice and does not bill insurance, Medicare or Medicaid for the primary care included in membership. Providers we refer you to, such as laboratories, imaging centers and specialists, may bill your insurance, Medicare or Medicaid as usual.

Direct Primary Care is not insurance.
Your Mountain Peaks membership pays for primary care delivered by this practice. It is not a health plan, not insurance, and not a substitute for insurance. Members should maintain appropriate coverage for hospitalization, emergency care, specialists and services outside the practice.
