Frequently asked questions

Straight answers about membership.

If something here is not clear, call the office at 801.724.9840 and ask. That is rather the point.

The model

What is Direct Primary Care?

Direct Primary Care is a membership model for everyday primary care. Instead of billing insurance for each visit, you pay Mountain Peaks a monthly membership. Removing insurance billing from routine primary care creates room for longer visits, easier access and a continuing relationship with your care team.

Is this concierge medicine?

No. There is no retainer on top of insurance billing and no exclusivity. It is one monthly membership that pays for the primary care this practice provides, priced to be workable for ordinary families.

Is membership health insurance?

No. Mountain Peaks membership is not health insurance. It covers primary care delivered by this practice. Members should maintain appropriate coverage for hospitalization, emergency care, specialists and services outside the practice.

Why is Mountain Peaks changing?

Because the appointment lengths and administrative load of insurance-billed primary care were limiting the medicine we want to practice. Dr. Durrans has written about this on the Current Members page.

Cost and coverage

How much does membership cost?

Adults up to age 64 pay $99 per month, adults 65 and older pay $110 per month, and children up to age 17 pay $25 per month under a parent's membership. A household membership is $299 per month and covers up to two parents and four dependent children through age 26; larger households should contact the office. Outside a household membership, dependents 18 to 26 pay the $99 adult rate. Membership payments qualify as an HSA expense, and FSA funds may qualify as well; check with your plan administrator. There is no enrollment fee, and if you prepay 12 months you receive a 1-month discount. Full details are on the Pricing page.

Do I still need health insurance?

In most circumstances, yes. Membership covers primary care here. Insurance protects you against the cost of hospital care, emergencies, specialists and imaging. Whether to carry coverage, and which plan to choose, is your decision, and we recommend talking to a licensed advisor.

Can I pay with an HSA or FSA?

Yes. Membership payments qualify as an HSA expense, so you can pay your Mountain Peaks membership from a health savings account. FSA funds may qualify as well; check with your plan administrator. Consult your own tax advisor about your individual circumstances.

What if I am on Medicare or Medicaid?

Membership works the same way for every member, whatever your coverage, including Medicare and Medicaid. Mountain Peaks is a membership practice and does not bill insurance, Medicare or Medicaid for the primary care included in membership. Providers we refer you to, such as laboratories, imaging centers and specialists, may bill your insurance, Medicare or Medicaid as usual.

Are labs and vaccines included?

Office visits are included. Laboratory testing, vaccines, dispensed medications and procedures are charged separately at transparent member pricing, quoted before you agree to them.

Access and care

Is there a mobile app for members?

Yes. Mountain Peaks members use the Mountain Peaks member app (iPhone and Android) to schedule visits, message their care provider, view records, prescriptions and lab results, join video visits, and manage membership billing. After each visit you also get a plain-language recap of what was discussed. The app is provided through SigmaMD, our secure membership platform; download instructions and sign-in details are sent to you when you enroll.

How quickly can I be seen?

Members can be seen the same or the next day when it is clinically appropriate. Availability depends on the day and on clinical need. Mountain Peaks does not promise unlimited or round-the-clock access.

How do I reach the practice between visits?

Between visits, members use the member app to contact Dr. Durrans directly with questions, follow-ups and refill requests. Messages are reviewed during office hours, and you will usually hear back the same business day, in between other patients. For anything urgent, call the office at 801.724.9840, and for an emergency call 911.

What do I do after hours or on the weekend?

Call the office line at 801.724.9840; outside office hours it forwards to the on-call phone. You can also send a message through the member app, and Dr. Durrans will respond as soon as is reasonable. For anything life-threatening, go to the emergency room or call 911.

What happens if I need a specialist or the hospital?

We make the referral, share the relevant records and stay involved. Care delivered by another practice or hospital is billed by them, usually to your insurance.

What should I do in an emergency?

Call 911 or go to the nearest emergency department. Membership does not replace emergency care.

Joining and leaving

How do I join?

Start on the Join the Waitlist page. Enrollment is handled through our secure enrollment system, which is where any personal or payment information is entered.

Can I cancel?

Yes. Membership is month to month with no long-term contract. You may cancel with 30 days' notice, or before your next billing date. Re-enrollment is always welcome; a $100 re-enrollment fee applies.

I am already a patient. What do I need to do?
